I/we hereby apply to Spike Design Australia Pty Ltd for credit accommodation and submit the following confidential information for this purpose.

l/we have never been bankrupt, nor have l/we ever been a Manager or Director of any company which has gone into liquidation, appointed an official manager, entered a scheme of arrangement with creditors or had a receiver and manager appointed.

I/we acknowledge Spike Design Australia Pty Ltd's conditions of service and also that payment shall be made by the due date listed on all invoices.

I / we acknowledge that accounts will be deemed overdue if payment has not been received by Spike Design Australia Pty Ltd by the due date. All accounts are placed on stop credit if payments have become and remain overdue more than 14 calender days.

I / we will enable Spike to gain first priority (or any other priority Security Interest is enforceable, perfected and otherwise effective under the PPS Law.

I /we agree to pay any debt collector's expenses for any amount entrusted to a collector, an interest of 5% of balances overdue, and to indemnify Spike Design Australia Pty Ltd for all other costs incurred by you howsoever arising from the collections of any monies due by me/us.
